Forms

To participate in athletics at San Marcos High School the following forms need to be completed.  Please read the Athletic Code with your athlete and sign that you have read and understand the code of conduct.  A physical needs to be completed before participating in any practices or games.  The Proof of Insurance and Consent to Treat a Minor Forms should be turned in with the physical.  Please complete the Driving Forms if you plan on transporting any athlete other than your own.  It is best to do these at the beginning of each year even if you’re not sure that you will be driving as they take over a week to clear the district office.  Please consider joining the athletic boosters as they pay for all transportation for the teams as well as fulfilling coaches wish lists.
